Procedure

Procedure about live in caregiver

Babs Zorg provides care providers who are live in with the client for 1 to 3 months. The care providers stay for a period of two or three months, after that the change takes place. We strive to ensure that they are two healthcare providers the same who replace each other every two or three months. In accordance with the Dutch Working Hours law, they work between 35 and 40 hours a week, spread over 6 days. They have room and board at your home, they can supervise you with their presence for the rest of the time, they are 1 day a week off.

In six steps to long-term resident care provider

1. Our services start with Free home visit. During this home visit we bring gladly together with you and your family your wishes, needs and the demand for care.

2. In this conversation we will look together to see whether a resident care provider offers a good solution.

3. During this meeting, our care manager will draw up your care and client profile. In consultation with the client and family, agreements are made about the desired services, these are recorded in a daily schedule and care plan and recorded in the care file.

4. When the care needer  gives us permission, we will develop and translate the intake form plus photo and make a job offer. This is forwarded to the care agencies we work with.

5. When the suitable caregiver has been found, she will then be tested for her suitability for the position and you will receive a CV. We advise you to get to know the care provider by means of (video) calling. This will give you a better picture of the healthcare provider.

If necessary, the care manager can advise you in making a choice.

6. When you agree to the arrival of the caregiver and the start date. We will organize the journey of the caregiver for you.

Model contracts

For the placement of a live-in care provider, we will make use of 2 model contracts.

Agreement 1

model contracts of the SVB (social insurance bank).

Agreement 2

Agreement between the budget holder and Babs Zorg Bureau.

Payment of the personal budget (PGB) and the Care Profile

Our services can always be paid for privately, but most clients are qualify for a personal budget (PGB) on the basis of their clinical picture and the related care level.

When a WLZ indication has been established, you can request a PGB.

The PGB budget covers the costs for healthcare in full from a minimum care indication ZZP 5.

The care severity profile (ZZP) is determined by the Care Assessment Center (CIZ).

A procedure must be followed to apply for a care severity profile (indication) and PGB.

Evaluation moments and safeguarding the quality of our services

Our organization considers it important to be regularly informed about the agreed services and to evaluate them.
Care must be provided professionally and responsibly.
The care manager is the first person of contact for the client or family. The healthcare provider will report daily on the progress of the healthcare provision. This report will be discussed during the evaluation with you as a customer and the care manager as an intermediary. In this way, the care manager can immediately answer questions from you or the care provider. Outside of the evaluation moments, the care manager can always be reached by telephone during office hours.
